2. Parent/Guardian Submissions

Registration and waiver forms are intended to be completed by a parent, legal guardian, or adult party host. Children should not submit these forms directly. If you believe a child has submitted a form without parent or guardian authorization, please contact us so we can address it.

3. Information Collected

When you submit a registration, waiver, or party request form, we may collect:

  • Child/student registration details (name, date of birth, program or school)
  • Parent/guardian contact details (name, phone number, email address)
  • Emergency contact details (name and phone number)
  • Medical and safety information (allergies, medical conditions, medications, physical limitations, and special requests/notes)
  • Private group details (group name, preferred location, preferred dates and times)
  • Party/event details (party name, event date, time, location, number of kids, age range, and notes)
  • Waiver and consent records (liability waiver acceptance, first aid/emergency medical consent, photo/video consent selection, party deposit acknowledgement, typed parent/guardian or host signature, waiver version)
  • Submission information (timestamp of form submission)
  • Signed waiver PDF records (if generated)

4. How Information Is Used

Information collected through our forms is used to:

  • Register students in school programs
  • Manage school program scheduling, enrollment, and attendance
  • Process and coordinate private group session requests
  • Process and coordinate soccer party requests and bookings
  • Create and maintain signed waiver records
  • Communicate with parents, guardians, and party hosts about their registration, waiver, booking, or program
  • Support participant safety, including using medical and emergency information when needed during sessions or events
  • Internal business recordkeeping
  • Insurance, dispute resolution, and legal record purposes

5. Medical and Emergency Information

Medical, allergy, and emergency contact information is collected to help support participant safety. This information may be shared with Coach Evan Canepa, authorized assistants or co-coaches, school or event site contacts, or emergency responders when needed for safety purposes.

We collect only what is submitted voluntarily through our forms. Medical fields are optional but encouraged when they are relevant to participant safety.

6. Children's Privacy

Kick It Kids Soccer LLC does not create accounts for children and does not knowingly ask children to submit personal information directly. Forms are designed for parent, guardian, or adult host completion.

Parents or guardians who wish to review, correct, or request deletion of information submitted about their child may contact us at evan@kickit.info. Requests are subject to legitimate business and legal recordkeeping needs.

7. Photo and Video Consent

Photo and video consent is a separate selection within the registration and waiver forms and may be accepted or declined. Participation is not conditioned on photo/video consent.

If consent is granted, photos or videos taken during classes, sessions, or events may be used for Kick It Kids Soccer LLC purposes such as the website, social media, flyers, program and business materials, or internal records, depending on the consent wording used in the form at the time of submission.

If consent is declined, we will make reasonable efforts to honor that selection.

8. How Information Is Stored

Form submissions may be stored in one or more of the following:

  • Google Sheets (for registration and waiver data)
  • Google Drive (for signed waiver PDF records, if generated)
  • Email notifications sent to Kick It Kids Soccer LLC
  • Website and backend systems used to receive and process form submissions

9. Service Providers

We use third-party service providers to help operate our website and process form submissions. These providers may receive information as part of that processing:

  • Vercel — website hosting and serverless form processing
  • Google Sheets and Google Drive — registration data storage and signed waiver document storage
  • Resend — email notification delivery

These providers are used for operational purposes only and are not authorized to use your information for their own purposes beyond what is required for the service.

10. Information Sharing

Kick It Kids Soccer LLC does not sell personal information. Information may be shared only in the following circumstances:

  • With Coach Evan Canepa, authorized assistants, or co-coaches for program operation and participant safety
  • With school or event site contacts as needed for program coordination
  • With service providers (as listed above) for the purpose of processing and storing registrations
  • With emergency responders when needed for participant safety
  • For insurance, dispute resolution, or legal purposes as needed
  • As required to comply with applicable law or to protect the rights, safety, or property of Kick It Kids Soccer LLC or others

11. Data Retention

Registration, waiver, safety, and booking records may be retained indefinitely or as needed for program administration, business records, insurance, dispute resolution, legal/safety purposes, and signed waiver documentation. If you have questions about records related to your child or your submission, please contact us.

12. Parent/Guardian Requests

Parents, guardians, and adult hosts may contact Kick It Kids Soccer LLC to request access to, correction of, or deletion of information that was submitted through our forms. 13. Security

Kick It Kids Soccer LLC uses reasonable administrative and technical measures through its service providers to help protect submitted information. However, no website or data transmission system is completely secure. We encourage you to submit only the information needed for your registration, waiver, or booking.
